| Each family at Our Lady's Academy must volunteer at least 25 service hours a year to be eligible for the service tuition discount. Of the 25 service hours, 10 hours must be directly related to marketing or fundraising activities. Activities you can volunteer for which will qualify for Service Hours are on the PTO page.
To record your service hours, you need to fill out these Service Hours Logsheets and submit them to Mrs. Minor at the end of November, January, and April. On the back of each log is an explanation of the new system for acceptable service hours.
All families are required to participate in Bingo and usual Bingo duties do not count towards service hours. |

School Buses and Parking Lot Procedure |

School Buses
Buses are provided free of charge by the city of Waltham to all Waltham residents who attend Our Lady's Academy in grades K through 8. (Buses are not provided for PreK students.) See the bus schedule for 2011-2012.
Parking Lot Procedure
For the safety of our children, we ask that all parents observe the rules of the parking lot when they are dropping off their children in the morning and picking them up after school. Please see the Parking Lot Procedures for an explanation and a diagram of the parking lot. |

School/Weather Closing |
In the case of inclement weather Our Lady’s Academy follows the “No School” decision of the Waltham Public Schools (if Waltham Public Schools are closed, then Our Lady’s Academy is closed; if Waltham Public Schools has a delayed opening, then the Academy has a delayed opening). Announcements are made on the major radio and television stations and their websites (WBZ, WCRB, WRKO, and Channels 4, 5, and 7) and The Waltham Cable channel. A two-hour delayed opening means that school starts at 10 a.m. and school buses pick up students on their routes two hours later than normal.
You can have an email sent to you to alert you about school closings by signing up with The Boston Channel. Remember to sign up for Waltham closings. School closings will not be posted on this website. |

School Nurse's Office |
| We have two nurses that provide coverage in our nurse's office: Andrea Coviello, LPN who is here on Monday and Tuesday from 8:30 to 2:30 P.M. and Christine Donaldson, RN who is here from 9:00 A.M. to 1:00 P.M. Monday through Friday. Our school nurses can be reached by calling the school office (781-899-0353) or the nurse's office directly (781-899-5027). |
Health Forms and Information The following forms need to be filled out and on file in order for our nurse to dispense medication. The Student Health and Emergency Information card must be returned on the first day of school. For more information on when it's okay to for a child to return to school after an illness, please see the guidelines below.

School Forms
The following forms have been updated for the 2010-2011 school year. All forms must be turned into the office on the first day of school. Some forms must be filled out at the office and they are not downloadable. (The forms listed here are those which should be filled out and returned to the School Office on the first day of school. - Student information form 2010-2011 -- We must have on file an updated list of emergency phone numbers for all parents and emergency contacts (if we are unable to reach the parents).

- CORI form (provided by the office and must be returned to the School Office with photo ID)

- Computer Network and Internet Policy (This document outlines the acceptable use policy governing students' use of the school network and Internet access)

- Student Network and Internet Agreement Form (All students and parents must sign this agreement form before a student is allowed to use the school network and access the Internet.)
